Dear Parents & Guardians,
This is a message from the health office.
New York State law requires health examinations for all students entering the school district for the first time and when entering Pre-K, K, 1st, 3rd, 5th, 7th, 9th, and 11th grade. The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner. A copy of the health examination must be provided to the school within 30 days from when your child first starts at the school, and when your child starts Pre-K, K, 1st, 3rd, 5th, 7th, 9th, and 11th grades. If you do not have a physical exam scheduled for your child, please do so as soon as possible. If your child has an up to date physical please provide a copy of the physical form to the health office as soon as possible. Forms can be faxed to 518-747-5196. Up to date immunizations are required for all grades. Please contact your health care provider to ensure all of your child’s vaccines are up to date before the start of school, and schedule any appointments necessary to comply with NYS vaccine law requirements for school entry. All vaccines are required to be up to date within the first 14 days of the start of the 2021-2022 school year.
