Philosophy: We at Fort Edward school believe that a healthy, safe and a well nourished child is the best learner.

Reminder NYS requires a physical exam for new entrants and students in grades pre-K or K, 1, 3, 5, 7, 9, and 11. NYS also requires physical exams annually for interscholastic sports, working papers, and as required by the Committee on Special Education (CSE). The date of examination must not be more than 12 months prior to the start of the school year and noted on the form. Immunizations should be updated yearly for all students.

Please scroll down for Health Forms.
